recently been discharged from a medical facility and require
continued therapy and/or medical care. The program follows through
with discharge plans, and also provides individualized assessments
and therapeutic care planning.

Buttonwoods Place Adult Day Health Center,
Inc. offers in-house nursing services provided by full time Registered
Nurses who plan, provide and supervise medical and nursing services
for each participant, as well as directing the care of the nursing
assistants. |
